Social Prescribing Vietnam (SPVN) welcomes collaboration with organizations interested in social prescribing, community-connected care, care navigation, social needs, implementation and evaluation in Viet Nam.

We work with partners to explore locally relevant approaches, strengthen implementation capability, generate practical learning and build more effective connections between healthcare and appropriate community resources.

Evidence, Education & Capability Building

Develop workshops, professional learning, evidence exchange and capability-building activities related to social prescribing, community-connected care, social needs and care navigation.

Social Needs & Community Resource Mapping

Explore social and practical needs, available community resources, access barriers, service gaps, capacity and opportunities to strengthen connections between healthcare and community support.

Care Navigation & Pathway Design

Explore locally appropriate navigation and referral pathways, including roles, responsibilities, community connections, follow-up, governance and escalation arrangements.

Implementation & Pilots

Assess readiness, co-design practical approaches and develop carefully governed pilots that can be tested, evaluated and improved before wider implementation.

Research, Evaluation & International Learning

Collaborate on research, measurement, implementation studies, evaluation and knowledge exchange, while adapting relevant international experience to the Vietnamese context.
