Evidence for Better Implementation

Evidence on social prescribing and community-connected care is growing, but findings vary across populations, settings, interventions and outcomes. Local context, community capacity, implementation quality and how outcomes are measured all matter.

Evidence & Insights brings together relevant research, implementation learning and informed analysis on social prescribing, community-connected care, care navigation, social needs and evaluation, with particular attention to what is relevant, and what still needs to be tested, in Viet Nam.

International Learning

What research suggests, and does not yet establish, about outcomes, mechanisms, equity, costs, measurement and evaluation.

Practical learning on pathways, care navigation, workforce, governance, safety, community resources, implementation and continuous improvement.

Perspectives on how social prescribing and community-connected care relate to Viet Nam's healthcare system, medical social work, population needs, community resources and local implementation conditions.

Relevant research, models, tools and implementation lessons from other countries, considered as sources of learning rather than models to be copied without local adaptation.

How We Approach Evidence

Evidence-Informed
Context-Sensitive
Implementation-Focused

We distinguish established evidence from emerging findings, implementation experience and informed perspectives. Where evidence is uncertain or mixed, we say so.

Evidence from one health system cannot automatically be assumed to apply elsewhere. Population needs, healthcare structures, community capacity, culture, resources and implementation conditions matter.

We look beyond whether an approach appears promising to how it can be implemented safely, equitably and sustainably, and how its effects can be measured and improved over time.

Building Knowledge for Viet Nam

As social prescribing and community-connected care develop in Viet Nam, Social Prescribing Vietnam will share relevant research, implementation learning, evaluation findings and practical resources.

Our aim is not to promote a predetermined model, but to support better questions, responsible implementation and locally relevant learning.
